License:Licensed in the state of Georgia in respective field. Required Minimum Experience: Minimum of 2 years experience in medical case managementinpatient rehab preferred; Demonstrated counseling skills, knowledge of community resources, federal and local guidelines, funding resources; understanding of adult and geriatric development and age appropriate care; ubnderstanding of the effects of illness and disability and the impact on both patient and family. Required Minimum Skills: Demonstrates skills or experience in case management. Strong computer skills required. Good communication skills both verbal and written. Excellent time management/organizational skills; working knowledge of the precertification process; Working knowledge of medical terminology and basic computer proficiency required The Case Manager is responsible for the oversightcoordination of quality patient care. The Case Manager facilitates team conferences, communication with families, and discharge planning. The Case Manager provides timely updates to insurance companiesadvocates for patients for needs both on the rehab unit and at discharge.
