Job Title: Case Manager On-Call Company: Marianjoy Rehabilitation Hospital Location: Wheaton, IL Description: The Case Manager serves as the primary liaison and educator for the patient/caregiver, and facilitates discharge coordination. The case manager communicates to external payers on behalf of patients admitted to the hospital. You will be responsible for building a partnership with the physicians to closely monitor utilization of the patient's resources and will work with the patient/family to plan and facilitate a timely discharge to home or to the appropriate level of care.Specific Responsibilities: The Case Manager handles a minimum caseload of 14 inpatients, following the patients from admission to discharge, as well as, outpatient appointments for follow-up. You will coordinate interdisciplinary team assessments, team conferences, review patient progress for medical necessity guidelines for continued stay and conveys plans to referring agencies to facilitate a smooth transfer process.
