Job Title: Social Worker/Case Manager; PT 2, Saturday & Sunday, 8am-4:30pm Company: Tenet Health Location: Sanford, NC Description: The Social Worker/Case Manager position is able to demonstrate the over all responsibilities of coordinating patient care activities to conform to evidence based practice and/or regulatory/payor requirements. This position integrates functions typically considered to be case managemnt, care coordination and/or utilization management as well as a discharge planning assessment. The post-discharge plans are initiated and completed through the use of the Extended Care Integrated Network (ECIN). The Social Worker/Case Manager assesses and documents psychosocial needs and interventions planned and/or taken as well as provides resources and makes referrals to community agencies for ongoing counseling and/or continued care. Maintains knowledge of community resources and regulatory requirements. Responsible for Performance Improvement, Patient Satisfaction, and goals and objectives established by management.
