Job Title: Case Manager Company: Bon Secours St. Francis Hospital - (Corp. 12) Location: Charleston, SC Description: Case Manager Case Management Salaried Full Time 8-4:30 Day shift Associates Degree or Bachelors Degree Required Job Summary: To be responsible for various aspects of care management to ensure that care being rendered is indicated, resources utilized are appropriate, and services provided are in the appropriate setting. Works collaboratively with the Case Management team, other departments and physicians to facilitate efficient and appropriate management of all cases. Minimum Qualifications: Education: Graduate of an accredited school, program or college of nursing, Social Work, Physical Therapy, Respiratory Therapy or Occupational Therapy or Masters degree in related field such as Clinical Counseling. Appropriate candidate will be a licensed RN or have a bachelor degree in the appropriate profession field. Experience: Minimum of 2 - 3 years of acute care experience within the last 5 years. Age Specific Competencies: Services are provided to inpatients and hospital based outpatients of all patient groups including neonatal, infant, child, adolescent, adult, and geriatric. Licensure/Certification, preferred: Appropriate licensure, certification or registration in the profession field of the applicant or currently licensed as a Registered Nurse in the state of South Carolina. Knowledge/Skills: Broad knowledge of all aspects of care coordination including but not limited to acute care criteria, progression of care through the continuum, discharge planning, social work issues, community health, welfare, social services agencies and programs. Other: Ability to develop and maintain positive relationships with physicians Ability to maintain confidentiality of sensitive patient information Ability to work in self directed manner
