I. Position Summary:
The Physical Therapist is responsible for the ethical and cost effective delivery of services as appropriate to their scope of practice for an Oklahoma state licensed Physical Therapist. II. Essential Functions:
The Physical Therapist responsibilities include, but are not limited to, the following:
* Assessment and/or evaluation of patients including interpretation of referral, development and modification of plan of care with goals and expected outcomes, and development of discharge plan
* Performs physical therapy interventions
* Documents of patient management prior to being filed in the permanent record.
* Provides education/instruction within areas of expertise and scope of practice to fellow employees, volunteers, students, and patient/clients.
* Acts as clinical instructor and may be required to evaluate student activities.
* Directs the physical therapy assistant, physical therapy students, and other personnel involved in patient care
III. Accountability:
The Physical Therapist is accountable and reports to the Director Physical Therapy or the Jim Thorpe Program Director.
IV. Qualifications:
* Graduation from an accredited institution in Physical Therapy
* Oklahoma Physical Therapist license
* Must be able to communicate effectively in English
* This job requires the incumbents to operate a INTEGRIS-owned vehicle OR personal vehicle (non INTEGRIS-owned) and therefore must have a current Oklahoma State Drivers License as well as a driving record which is acceptable to our insurance carrier.

Driving rehabilitation: exposed to traffic and weather conditions while riding with patients who are driving or from driving to and from potential clients home, high stress level potential, potential for involvement in accidents that may cause property damage and injury, frequent exposure to weather extremes.
