Job Title: Music Therapist Needed for Home Health & Hospice care Company: Location: Oklahoma City, OK Description: For immediate consideration, apply online at http://longtermcare.jobdigtracker.com/careers/?p=applyToJob&ID=2112&rq=5vetnjq7ln2m Music Therapist Job Description: The Music Therapist is part of the Interdisciplinary Hospice Team and assesses patients and families and collaborates with other team members to develop and implement the hospice Plan of Care, providing therapuetic interventions to meet the physical, emotional, and spiritual needs of hospice patients. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses the need for and provides therapeutic care services to hospice patients and their families. As a member of the Interdisciplinary Group (IDG), participates in the development of the Plan of Care. Attends IDG meetings. Documents all visits with patients and families according to Companion Hospice policies, procedures, and guidelines. Assumes and maintains accountability for music therapy program in compliance with the American Music Therapy Association (AMTA). Assists in the admission process as required. Participates in staff training and education programs. Develops and Implements Music Therapy Support Groups in selected communities. Performs education and training activities for staff and community groups, such as senior centers, discharge planners, etc...erforms other duties as assigned. MINIMUM QUALIFICATIONS: - Bachelor?s degree in music therapy from an approved University, including completion of approved internship. - Board certified or qualified to sit for the national exam. - Membership and registration in the American Music Therapy Association preferred. - Knowledgeable in cultural, ethnic and spiritual diversity. - Ability to work well independently and within the interdisciplinary team setting. - Demonstrates excellent oral, written and interpersonal communication skills. - Ability to work a flexible schedule. - Reliable transportation, current driver license and current auto insurance.
