Our local homecare office is currently seeking a Certified Occupational Therapist who, in collaboration with the multi-disciplinary team, develops a plan of care for each occupational therapy patient and provides skilled occupational therapy services in accordance with physician orders. Responsibilities: Provide skilled occupational therapy services/interventions in accordance with physician orders Assess patient needs, plan for, evaluate and modify care to meet goals of Occupational Therapy interventions Collaborate with all disciplines to plan and evaluate team goals for each patient Instruct patient/family/caregivers on patient's total occupational therapy program Advise, consult and instruct members of the multi-disciplinary team in specific occupational therapy modalities Adhere to all Maxim policies and procedures while providing services to Maxim patients Adhere to all Maxim expectations with regards to preparation and submission of required paperwork Observe and report patient progress and/or changes in patient status to physician and appropriate multi-disciplinary team members Participate in Multi-disciplinary Patient Care Conference, PI Committees, Professional advisory board and Maxim in-service programs Requirements include: Graduated from a school of Occupational Therapy accredited with the American Occupational Therapy Association and the Council on Medical Education of the American Medical Association Maintains certification through the American Occupational Therapy Certification board Minimum of one (1) year experience in an acute care facility, out-patient setting, community health service or Home Health agency in the past two (2) years Current malpractice insurance with a minimum of $1,000,000 each occurence/ $3,000,000 aggregate in malpractice liability Current CPR card Current Health Certificate (physical examination and PPD of Chest X-Ray) Willingness to enter into an Independent Contract Relationship and adhere to all Maxim expectations of Independent Contractors Reliable transportation Our company is committed to maintaining a challenging environment that promotes personal accountability, personal growth, and an active role in the driving vision of the company.
