JOB SUMMARY

The primary responsibility of the Physical Therapist (PT) is to plan, delegate and provide safe and competent physical therapy interventions as ordered by physician.

ESSENTIAL FUNCTIONS

  • Provide physical therapy assessments, as well as develop and implement individualized treatment plans to increase client’s independence and functioning
  • Supervise Physical Therapy Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standards
  • Identify need to make referrals to other disciplines to address comprehensive care needs of the client
  • Demonstrate competent technical skills according to current Physical Therapy standards of practice
  • Communicate changes in resident’s condition
  • Participate in discharge and community mobility planning
  • Prescribe necessary durable medical equipment to facilitate independence
  • Provide accurate and timely billing records and supervise supporting clinical documentation
  • Respond to family requests and concerns timely
  • Assure resident abuse or neglect does not occur
  • Observe confidentiality and safeguards for all patient information
  • Participate in collaborative care conferences
  • Coordinate and provide training with Restorative Nursing and other Care staff
  • Provide education and training to patient, caregiver, and community partners
  • Assist with training and education of facility and professional partners
  • Ensure compliance with mandatory education, training and competency programs
  • Performs other duties as assigned

JOB QUALIFICATIONS

  • Education
    • Graduate of an accredited physical therapy school.
  • License
    • Current Wyoming Physical Therapy License
  • Certifications required
    • See Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
  • Experience
    • Experience in providing appropriate care in post-acute or long-term care environment preferred

Knowledge, Skills, and Abilities:

  • The ability to maintain continuing education requirements and all certifications and licenses as required to maintain professional license.
  • Licensed professionals must strictly adhere to the ethical code of their respective discipline.
  • Must use good judgment in interpreting patient referrals and performing appropriate physical therapy procedures.
  • Ability to develop positive interaction with patients, patient families, physicians, and staff in order to effectively care for the patients.
  • Must be able to assess a patient’s condition, formulate a plan of care, select appropriate interventions, and evaluate patient response to care/treatment and to explain/teach patients about their condition/recovery.
  • Ability to perform multiple tasks simultaneously with frequent interruptions.
  • Analytical and reasoning skills with a high level of mental concentration and memory.
  • Ability to speak clearly to communicate and interact positively with patients, patient families, physicians, and staff in order to effectively care for the patients.
  • Computer Knowledge including the ability to enter material into electronic record. Ability to operate fax, computer, and copy machine to perform the duties of the position.
  • The ability to demonstrate knowledge of developmental stages and apply developmental theories/concepts when planning and implementing care for the appropriate age of the patient. Ability to maintain the appropriate level of knowledge and skills.
