Job Summary:
We are seeking a dedicated Home Care Physical Therapist to join our team. As part of Trinity Health at Home, you will have the opportunity to make a positive impact by providing care to patients in the comfort of their homes. You will deliver high-quality therapy, design individualized treatment plans, and support patients in regaining independence and mobility.

Responsibilities:

• Assess, plan, and administer physical therapy treatments for patients in their homes.

• Develop personalized treatment goals based on patients' individual needs.

• Educate patients and caregivers on therapeutic exercises and safe mobility practices.

• Coordinate with other healthcare team members to ensure comprehensive patient care.

• Document treatments and progress accurately in accordance with regulatory and company standards.

Requirements:

• Current State of Michigan licensure as a Physical Therapist.

• Minimum of 2 years of experience in physical therapy, preferably in a home health or related setting.

• Strong communication and organizational skills.

• Ability to work independently and manage a caseload in a home setting.
