The Home Care SLP is responsible for independent management of the Home Care patient population requiring use of advanced assessment, teaching, and decision-making skills. The Home Care SLP cares for a caseload of home care patients by evaluating the patient for appropriateness of home care needs and developing the plan of care in collaboration with the provider, caregivers, and Home Care IDG members. The Home Care SLP provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The Home Care SLP provides patient care based on providers orders, departmental policies and procedures, regulatory guidelines, and best practice. The Home Care SLP documents in the patients medical record in a timely manner according to agency policy. The Home Care SLP attends and actively participates in Case Conference and team meetings. The Home Care SLP performs on-call responsibilities. Required Education:
Masters Degree in Speech Language Pathology
Required Skills:
Therapy skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Professional interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (i.e., copy machine, fax machine, etc.) Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illness. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Preferred Skills:
Experience with Home Care Home Base; Experience with smart phones and smart devices

Required Licensure/Registration/Certification:
Current Texas license as a Speech Language Pathologist; CPR; current drivers license and auto insurance. Must have reliable transportation.
Preferred Licensure/Registration/Certification:
Advanced skills or certifications in specialty areas, such as VitalStim certification
Required Experience:
Work (or) educational experience in the healthcare field

Preferred Experience:
One year experience as a Speech Language Pathologist in a health care setting.
