Job Details:

• Duration: 12 weeks

• Hours: 40 hours/week, Monday-Friday

• Facility: One main location with minimal travel to nearby outpatient settings

• Population: Adults 60+, post-acute recovery

Key Responsibilities:

• Conduct patient assessments and set functional goals

• Implement individualized therapy plans to improve mobility and function

• Monitor and document progress

• Participate in care team meetings to ensure integrated rehab strategies

What You Need:

• Valid Utah PT License

• Experience in post-acute or SNF settings is highly valued

• Ability to thrive in a fast-paced rehab environment
