Are you a skilled Physical Therapist (PT) looking for a short-term opportunity in Provo, UT ? Were seeking a dedicated PT to provide full-time services in a post-acute care setting supporting adults aged 60 and up. This role is ideal for professionals with SNF or geriatric experience who are ready to make a meaningful impact.

Position Details:

• Start Date: ASAP

• Duration: 12 weeks

• Schedule: Monday Friday, 40 hours/week

• Location: One main facility with possible nearby outpatient visits

• Setting: Post-acute care/rehabilitation

Responsibilities:

• Evaluate patient conditions and develop custom treatment plans

• Deliver evidence-based physical therapy to older adults recovering from surgery or illness

• Document patient progress and adjust care plans accordingly

• Work closely with physicians, nurses, and other therapy staff

Requirements:

• Active UT Physical Therapist License

• Prior experience in SNF, post-acute, or geriatric rehab settings preferred

• Strong communication and organizational skills
