Manages and provides skilled physical therapy to patients/clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age related needs. Requires skills in patient/client evaluation and treatment, as well as professional consultations to achieve maximum patient/client independence in accordance with the individuals functional level and potential.

Consequently, the employee must be competent in the interpretation of a patient's self-report or behavior, and this information must be interpreted with an understanding of the cognitive, physical, emotional/psychosocial and chronological maturation process. The treatment of patients should be individualized and should reflect an understanding of the developmental needs and range of treatment needs for each patient.
Position Specific Responsibilities
- Assesses patients.
- Plans and administers medically prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body part.
- Completes comprehensive physical assessments and identifies need for referral to additional appropriate clinical disciplines.
- Performs patient tests, measurements, and evaluations such as range-of-motion and manual muscle tests, gain and functional analysis, and body-part measurements.
- Records and evaluates tests, measurements, and evaluations to aid in establishing or revising specifics of treatment programs.
- Administers manual therapeutic exercises to improve or maintain muscle function.
- Instructs, motivates, and assists patients in non-manual exercises, such as isometric and progressive-resistive exercises; and in functional activities, using weights, pulleys, exercise machines, amts, steps, and assistive and supportive devices such as crutches, canes, parallel bars, and prostheses.
- Administers treatments involving applications of physical agents such as heat/cold, massage and other modalities under a physicians order.
- Confers with physician and other health practitioners to obtain additional patient information, suggest revision in treatment programs, and integrate physical therapy with other aspects of patient health care.
- Instructs patient and family in physical therapy procedures to be continued at home.
- Supervises licensed physical therapy assistants and aides.
Education and/or Experience
- Required - Bachelor's Degree From an accredited Physical Therapy Program Or
- Required - Master's Degree From an accredited Physical Therapy Program.
- Required - D.P.T.
- From an accredited Physical Therapy Program.
- Required - 1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.
Skills
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
Certifications, Licenses, and Registrations
- Required upon hire: Washington Physical Therapist License (Vendor Managed)
- Required within 30 days of hire: National Provider BLS - American Heart Association (Vendor Managed)
- Required within 30 days of hire: National Provider Identifier (NPI) number
Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential.
