Patient Care Coordinator Job Summary:

The Patient Dining Associate plays a key role in delivering personalized, accurate meal service to patients and residents, ensuring dietary needs and preferences are met with a high standard of customer service. This position supports daily food service operations by coordinating meal orders, delivery, and communication across departments

Patient Care Coordinator Responsibilities:

  • Utilize handheld tablets to take personalized meal orders from patients and residents, ensuring accuracy, and adherence to dietary restrictions. Input and process food and beverage orders using the Point of Sale system. Review and verify daily meal orders for completeness and correctness.

  • Deliver trays to patient and resident rooms or units as assigned; Ensure deliveries are timely and trays are accurate. Ensure highest level of customer service during each delivery.

  • Maintain and update MealTracker patient profiles, including new admissions, dietary changes, and allergen information.

  • Provide production numbers to operations for the next day's meal preparation.

  • Collaborate with the hospitality coordinator to communicate information to the food service team.

  • Ensure staff checkout procedures are completed at the end of each shift to meet both Nexdine and client expectations

  • Monitor and respond to department voicemail, including call-offs and resident feedback/concerns

  • Print and distribute weekly menus, ensuring each patient receives the correct menu based on their diet order.

  • Assist with tray audits and IDDSI compliance checks.

  • Serve as the primary point of contact for staff, residents, and nursing regarding meal-related inquiries.

  • Promote a safe work environment by adhering to company safety and security policies.

  • Maintain organized and clean work areas, including computers and phones.

  • Ensure Department phones are covered at all times, coordinating breaks among trained staff

Requirements:

  • High School diploma or equivalent.

  • Minimum of 1 year of experience in a similar role, preferably in a healthcare setting.

  • Excellent communication and interpersonal skills.

  • Ability to lift up to 50 pounds and stand for extended periods.

  • Familiarity with dietary guidelines and allergen awareness.

  • Proficiency in using handheld devices and Point of Sale (P.O.S.) systems.

  • Strong organizational skills and attention to detail.
