Hospice Music Therapist

JOB REQUIREMENTS: Overview PRN Coordinates, evaluates and provides musictherapy to patients receiving hospice care. Responsibilities Develops,coordinates and provides an organized music therapy program. Providesmusic therapy in alignment with the needs of the patient and physicallimitations. Develops a plan of care. Completes all documentation asrequired by the organization in compliance with legal, governmental andaccreditation requirements. Participates in quality assessment andperformance improvement activities. Performs other job-related tasks asrequested. Qualifications Required- Certified music therapist from anestablished music therapy institute. Licensed driver with reliableautomobile which is insured in accordance with state law.
