SummaryThe Occupational Therapy Assistant provides home care services in community based and facility settings. The position is responsible for providing therapy services to patients enrolled in home health and hospice programs under the direction of the patient’s physician and with supervision of an Occupational Therapist for treatment of a patient's condition as it relates to the patient's disabilities, injuries and/or disease process. Quality of care is provided in accordance with state and federal regulations and the mission of Home Care Services.
Position Reports To: Clinical Manager and agency leadershipEssential Functions
- Follows a patient specific plan of care established by an occupational therapist to help patients develop, regain or maintain their ability to perform daily activities in their home environment and community.
- Administers skilled treatment interventions as prescribed by the physician that may include but are not limited to utilization of therapeutic exercises/activities, modalities, design/fabrication/recommendation of adaptive equipment /orthotics and assistive devices, compensatory techniques and instructing the patient/family in home programs to be continued in the therapist’s absence, when indicated.
- Communicates with the supervising occupational therapist the patient's progress of the following (but not limited to): activities of daily living, instrumental activities of daily living, cognitive skills, pain, visual perception, neuromuscular and sensory integrative skills and abilities, functional needs and abilities, activity tolerance, mobility, positioning, safety in the home environment, equipment needs, psychosocial status, need for community resources and home health/hospice services.
- Assists in coordinating the delivery of occupational therapy services with nursing, social work, aide, therapy, equipment suppliers, community resources and transitions of care which results in effective utilization of resources and timely attainment of achievable rehabilitation and/or palliative care goals.
- Participates in weekly interdisciplinary team meetings and agency meetings as assigned.
- Communicates with occupational therapist the need to report to physician the patient’s reaction to treatment, any changes in the patient’s condition, suggested changes in the patient’s orders and implementation of discharge planning procedures.
- Maintains documentation of subsequent visits and coordination of care and other clinical record documentation in the electronic medical record.
- Instructs and supervises other health team members in occupational therapy procedures and techniques performed as an adjunct to the regular occupational therapy plan of care.
- Required to maintain active Basic Life Support (CPR) certification.
- Other duties as assigned.
Qualifications
Required:
- Graduate of an AOTA accredited occupational therapy assistant program.
- Current License with State of Washington as an Occupational Therapy Assistant.
- Communication: Functionally able to listen, read, speak and write using English language. Functional hearing and visual acuity sufficient to detect changes in patient conditions and perform assessments. Effective interpersonal skills. Demonstrates ability to work in collaboration with health team members.
- Must possess basic computer/electronic device skills related to Windows and app navigation, mouse and screen usage, keyboarding, email communication and password management.
- Current, valid driver's license and access to an insured vehicle with the minimum coverage by law.
Desired :
- One year of recent occupational therapy assistant experience.
- Previous home care, acute care or skilled nursing experience.
