Post Acute Case Manager

The Post Acute Care Management (PACM) Department is seeking an RN to join our team as a PACM Liaison.

This is not a hands-on patient care position, however, as a Liaison you will use your skills as a nurse to evaluate patients for post-acute rehab needs and help them get into one of our 3 skilled nursing facilities or our acute inpatient rehab.

The right candidate for this position will be an RN with at least 2 years of experience, preferably in nursing. You must be outgoing, detail-oriented, and work well autonomously as well as with a team.
