Location: Tulsa, OK Description: Job Title: Home Health Physical Therapist Duration: 13-week contract Schedule: Day Shift, 8 hours per day (Monday to Friday) Location: Tulsa OK (Home health, specific territory TBD) Job Summary: As a Home Health Physical Therapist, you will have the rewarding opportunity to work directly with patients in their own homes. Your role will be vital in helping patients regain their strength and independence, allowing them to continue living in their homes and actively participate in their desired daily activities. By providing personalized care and therapy, you will witness firsthand the positive impact you can make on your patients' lives. Responsibilities: Conduct one-on-one physical therapy sessions with patients in their homes Develop individualized treatment plans to address patients' specific needs and goals Assist patients in improving their mobility, strength, and functional abilities Evaluate patient progress and modify treatment plans as necessary Educate patients and their families on proper techniques and exercises for ongoing improvement Coordinate care with other healthcare professionals to ensure comprehensive treatment Maintain accurate and detailed documentation of patient evaluations, progress, and outcomes Minimum Qualifications: Bachelor of Science in Physical Therapy Current state license as a Physical Therapist Valid driver's license for home visits Current CPR certification Negative TB screen and Hepatitis consent/declination Two years of experience as a Physical Therapist in an acute care or rehabilitation setting Knowledge and Skills: Proficiency in therapy skills as defined by generally accepted standards of practice Excellent interpersonal skills to build rapport with patients and their families Knowledge of durable medical equipment commonly used in home health settings Join our team of dedicated professionals and be part of a rewarding journey that positively impacts patients' lives in the comfort of their own homes.
