Vocational Case Manager

  • Observe clients in their work area and provide one-on-one job coaching, assistance, and direction as needed. Will also assist supervisor(s) with addressing client issues as they arise in the workplace.
  • Identify and implement necessary accommodations to enable consumers to complete their assigned jobs. Assess and implement necessary assistive technology to address individuals specific needs.
  • Participate in the individuals team meetings, providing updates regarding vocational progress.
  • Coordinate and participate in interdisciplinary team meetings as needed or requested by members of the individuals support team.
  • Counsels consumers to promote productive work attitudes and skills.
  • In conjunction with the consumer, develop appropriate vocational outcomes based on the individuals qualifications, interests, skills, and support needs.
  • Provide individual and crisis counseling as needed.
  • Identify additional services and resources that the individuals may need. Make recommendations/referrals as appropriate.
  • Develop and supervise downtime activities and lunches.
  • Assist with caseload coverage as needed.
  • Assist in planning, coordinating, and supervising annual client parties and events.
  • Occasionally may need to transport individuals with disabilities to and from their residence, hospital/workers compensation visit and to and from various work/contract assignments using their own vehicle when an OTC vehicle is not available.
  • Complete reports in a timely manner including annual evaluations/assessments, monthly/quarterly statistic reports, individual goal plans, injury/illness and unusual incident reports, and other reports as necessary or assigned.
  • Oversee the proper administration of medication, including compliance with all procedures and recordkeeping.
  • Establish and maintain a professional level of communication with OTCs administration, workshop personnel, program consumers, parents, guardians, residential staff/personnel, families, and other stakeholders as necessary.
  • Willingly and effectively cooperates with OTC, The Department of Human Services, The Division of Developmental Disabilities (DDD), The Division of Vocational Rehabilitation Services (DVRS) and other licensing, state agency or local municipalities in any inspections and investigations, upon request
  • Other duties as assigned.
  • BA or BS in Human Services, psychology, sociology or related field
  • 21 years or older
  • Minimum of two (2) years counseling/vocational experience working with individuals with disabilities.
  • The successful completion of all DDD required training and compliance with all applicable standards as set forth in the manual upon the date of hire.
  • Ability to perform CPR and First Aid on ground level.
  • Availability 8AM 4PM, hours may vary.
  • Valid driver license with acceptable driving record.

Necessary Special Requirements :

  • Because this job is considered safety sensitive, a drug screen test will be required prior to employment, at a random basis thereafter and for cause.
  • Employee should agree to have their name verified against the Central Registry of offenders against individuals with developmental disabilities.
  • Employee must complete an application to have their name checked against the child abuse registry (CARI checks).
