Location: Burlingame, CA
Job Type: Full-Time, 13-Week Contract (with potential extension)
Schedule: MondayFriday, 8:00 AM 5:00 PM

Overview:
Sunbelt Staffing is seeking a licensed and experienced Physical Therapist (PT) for a 13-week contract opportunity in a Home Health setting in Burlingame, California . This role involves providing skilled physical therapy services in patients homes to promote recovery, improve mobility, and enhance quality of life. Candidates should be motivated, independent, and passionate about delivering high-quality, patient-centered care.

Key Responsibilities:

• Perform comprehensive patient evaluations and develop individualized treatment plans

• Deliver physical therapy interventions to restore function and improve patient outcomes

• Monitor and document patient progress, adjusting plans as needed

• Collaborate with caregivers and interdisciplinary team members

• Travel to patient homes within the local area (mileage reimbursed)

• Ensure accurate and timely documentation according to agency and regulatory standards

Qualifications:

• Graduate of an accredited Physical Therapy program

• Active PT license in the state of California

• Valid drivers license and reliable transportation

• Basic Life Support (BLS) certification required

• Prior Home Health experience preferred

• OASIS experience preferred

• Familiarity with Home Care Home Base (HCHB) EMR preferred

• Strong clinical, organizational, and communication skills

Compensation & Benefits:

• $55$70/hour based on experience
