Physical Therapist (PT) Home Health

Location: Phoenix, AZ
Job Type: Full-Time, 13-Week Contract (with potential extension)
Schedule: MondayFriday, 8:00 AM 5:00 PM

Overview:
Sunbelt Staffing is seeking a licensed and experienced Physical Therapist (PT) for a 13-week contract opportunity in a Home Health setting in Phoenix, Arizona . This role involves providing skilled physical therapy services in patients homes to restore function, improve mobility, and enhance quality of life. Candidates should be self-directed and passionate about patient-centered care.

Key Responsibilities:

• Perform patient evaluations and develop individualized treatment plans

• Deliver physical therapy interventions to improve patient outcomes

• Monitor and document patient progress, adjusting plans as needed

• Collaborate with caregivers and interdisciplinary team members

• Travel to patient homes within the local area (mileage reimbursed)

• Maintain accurate and timely clinical documentation per agency and regulatory standards

Qualifications:

• Graduate of an accredited Physical Therapy program

• Active PT license in the state of Arizona

• Valid drivers license and reliable transportation

• Basic Life Support (BLS) certification required

• Prior Home Health experience preferred

• OASIS experience preferred

• Familiarity with Home Care Home Base (HCHB) EMR preferred

• Strong clinical skills and excellent communication abilities

Compensation & Benefits:

• $55$70/hour based on experience
