Location: Portsmouth, NH
Job Type: Full-Time, 13-Week Contract (with potential extension)
Schedule: MondayFriday, 8:00 AM 5:00 PM

Overview:
Sunbelt Staffing is seeking a licensed and experienced Physical Therapist (PT) for a 13-week contract opportunity in a Home Health setting in Portsmouth, NH . This position offers the chance to provide personalized physical therapy care in patients homes, supporting their recovery and functional independence. Ideal candidates are self-directed and passionate about delivering high-quality care.

Key Responsibilities:

• Conduct comprehensive patient evaluations and create individualized treatment plans

• Provide physical therapy services to improve mobility, strength, and function

• Monitor patient progress and adjust treatment plans accordingly

• Collaborate with caregivers and interdisciplinary team members

• Travel to patient homes within the local area (mileage reimbursed)

• Ensure accurate, timely documentation following agency and regulatory standards

Qualifications:

• Graduate of an accredited Physical Therapy program

• Active PT license in the state of New Hampshire

• Valid drivers license and reliable transportation

• Basic Life Support (BLS) certification required

• Previous Home Health experience preferred

• OASIS experience preferred

• Experience with Home Care Home Base (HCHB) EMR preferred

• Strong clinical and communication skills
