Occupational Therapist (OT) Home Health

Location: Cleveland, OH
Job Type: Full-Time, 13-Week Contract (with potential extension)
Schedule: MondayFriday, 8:00 AM 5:00 PM

Overview:
Sunbelt Staffing is seeking a licensed and experienced Occupational Therapist (OT) for a 13-week contract opportunity in a Home Health setting in Cleveland, OH . This position offers the opportunity to provide one-on-one, in-home therapy to patients, helping them regain independence and achieve optimal daily functioning. The ideal candidate is organized, communicative, and confident working independently in the field.

Key Responsibilities:

• Perform initial and ongoing patient assessments

• Develop and implement individualized treatment plans

• Deliver occupational therapy services to support activities of daily living

• Document patient progress and adjust care plans as needed

• Collaborate with the interdisciplinary care team and caregivers

• Travel to patient homes in the local area (mileage reimbursed)

• Maintain timely, accurate clinical documentation

Qualifications:

• Graduate of an accredited Occupational Therapy program

• Active OT license in the state of Ohio

• Valid drivers license and reliable transportation

• Basic Life Support (BLS) certification required

• Previous Home Health experience preferred

• OASIS experience preferred

• Familiarity with Home Care Home Base (HCHB) EMR preferred

• Strong communication and time management skills
