Job Type: Full-Time, 13-Week Contract (with potential extension)
Schedule: MondayFriday, 8:00 AM 5:00 PM

Overview:
Sunbelt Staffing is seeking a licensed and experienced Physical Therapist (PT) for a 13-week contract opportunity in a Home Health setting in Owensboro, Kentucky . This role offers the opportunity to provide in-home physical therapy services focused on improving patient mobility, function, and quality of life. The ideal candidate is detail-oriented, motivated, and committed to delivering excellent patient care.

Key Responsibilities:

• Perform thorough patient evaluations and develop individualized treatment plans

• Deliver physical therapy interventions aimed at restoring patient function

• Monitor and document patient progress, making adjustments as needed

• Collaborate with caregivers and interdisciplinary team members

• Travel to patient homes within the local area (mileage reimbursed)

• Maintain accurate and timely clinical documentation per agency and regulatory standards

Qualifications:

• Graduate of an accredited Physical Therapy program

• Active PT license in the state of Kentucky

• Valid drivers license and reliable transportation

• Basic Life Support (BLS) certification required

• Prior Home Health experience preferred

• OASIS experience preferred

• Familiarity with Home Care Home Base (HCHB) EMR preferred

• Strong clinical, organizational, and communication skills
