Location: Cheraw, SC
Job Type: Full-Time, 13-Week Contract (with potential extension)
Schedule: MondayFriday, 8:00 AM 5:00 PM

Overview:
Sunbelt Staffing is seeking a licensed and dedicated Occupational Therapist (OT) for a 13-week contract position in a Home Health setting in Cheraw, SC . This opportunity is ideal for an OT who thrives in an autonomous environment and is passionate about helping patients regain independence through personalized, in-home care.

Key Responsibilities:

• Conduct initial and ongoing patient assessments

• Develop and implement individualized treatment plans

• Provide occupational therapy interventions focused on daily living and functional mobility

• Monitor and document patient progress and outcomes

• Collaborate with caregivers and interdisciplinary team members

• Travel to patient homes within the service area (mileage reimbursed)

• Maintain accurate, timely documentation in accordance with agency policies

Qualifications:

• Graduate of an accredited Occupational Therapy program

• Active OT license in the state of South Carolina

• Valid drivers license and reliable transportation

• Basic Life Support (BLS) certification required

• Previous Home Health experience preferred

• OASIS experience preferred

• Familiarity with Home Care Home Base (HCHB) EMR preferred

• Strong communication, organizational, and time management skills
