Your Role:
Your primary role is to work collaboratively with your team to provide direct rehabilitation care in speech therapy to residents upon physician referral as well as to adequately plan for resident needs.
Meet the resident's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining speech therapy treatment plans in consultation with physicians or by prescriptionAdminister speech therapy treatmentsComplete discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to resident care conferencesAssure continuation of therapeutic plan following discharge by instructing residents, families, and caregivers in home care programs; recommending and/or providing assistive equipment; recommending home health or follow-up programs
What We're Looking For:
You will be able to demonstrate composure, customer focus, patience, and the ability to gain trust and establish rapport with residents and your team.
A strong desire and passion to work in an environment with people that are unwell, disabled, or living with dementia. Prior experience with mental health and psych population is preferred.Must be a graduate of an accredited program in speech therapy (minimum MSSLP)Must be ASHA certified (CCCs) or must be eligible for 9-month Clinical Fellowship Year (CFY) in order to earn ASHA certification (CCCs) and/or meet ASHA credentialing criteriaOne year's experience in long-term care or related setting preferred
