Speech Language Pathologist, SLP Job Description:

The Clinical Fellow (CF) Speech-Language Pathologist is a graduate-level practitioner who works under the supervision of a licensed and experienced Speech-Language Pathologist (SLP). The CF is responsible for assisting in the evaluation, diagnosis, and treatment of residents with speech, language, cognitive-communication, and swallowing disorders. This position is designed to help the Clinical Fellow gain the necessary experience and skills to complete the required clinical fellowship hours as mandated by the American Speech-Language-Hearing Association (ASHA) and earn full licensure as a Speech-Language Pathologist.

Speech Language Pathologist, SLP Responsibilities:

Assessment and Evaluation: Conduct thorough assessments of residents' speech, language, cognitive, and swallowing abilities.

Individualized Care Plans: Develop customized intervention plans to address communication and swallowing difficulties.

Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments, as applicable.

Treatment: Administer speech therapy interventions, including exercises, communication strategies, and swallowing therapy.

Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service (POS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).

Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.

Interdisciplinary Collaboration: Collaborate with other rehab staff, such as Physical Therapists and Occupational Therapists, to provide holistic care for residents.

Clinical Fellow Speech Language Pathologist SLP Qualifications:

Master's Degree in Speech-Language Pathology from an accredited program.

Current Clinical Fellow (CF) status or eligible to begin CF.

Must be licensed or eligible for licensure as a Speech-Language Pathologist in NJ.

Strong written and verbal communication skills.

Ability to work effectively in a multidisciplinary team.
