The Occupational Therapist (OT) assists individuals of all ages to develop, recover, and improve the skills needed for daily living, leisure, and working. Assesses patient conditions and deeded/desired daily occupational functions, designing customized therapeutic interventions and implementing rehabilitation programs to enhance independence and quality of life. Collaborates with a multidisciplinary healthcare team to create holistic care plans, provides patient education, and adapts environments to support patient's functional abilities.
Duties/Responsibilities:
Performs comprehensive evaluations of patients' physical, mental, and environmental factors to determine limitations and strengths related to daily living activities to develop an appropriate and individualized treatment plan to address each patient's needs.
Provides appropriate and evidenced-based therapeutic interventions by implementing therapeutic exercises/activities, adaptive equipment, and environmental modifications to enhance patients' physical and cognitive functioning.
Assess patient progress, adjust treatment plans, and collaborate with the appropriate healthcare team members to ensure optimal recovery and outcomes.
Educates patients and caregivers and guides a home program, including exercises, modifications to promote independence, and training on adaptive tools/equipment, including strategies to achieve long-term health improvements for daily living based on the patient's disability or developmental delays, age, and psychosocial needs.
Collaborates with referring providers, nurses, other rehab disciplines, and other healthcare team members to ensure coordinated and comprehensive care.
Maintains accurate and up-to-date medical records and documentation timely.
Supervises and instructs rehabilitation techs as necessary.
Education:
Graduate of an Occupational Therapy (OT) program required.
Experience:
One year of clinical experience preferred.
Licensure/Certification:
Valid Kentucky Occupational Therapy (OT) License required. Basic Life Support (BLS) Certification within 90 days of the hire date required. Valid Kentucky driver's license (if home health) required.
