Location: Murrieta, CA
Work Setting: Hybrid (Field-based with administrative flexibility)
Schedule: MondayFriday, 8:00 AM 5:00 PM
Some Saturday availability required (810 times/year)
Compensation: $55$60 per hour
Travel Required: Yes (valid drivers license, insurance, and reliable vehicle required)

Position Summary
As a Physical Therapist (PT) in the Home Health team, you will plan, coordinate, and deliver rehabilitative services to patients in their homes. Your work will emphasize community-based care, education, and personalized treatment strategies that support patient independence, mobility, and quality of life.
Key Responsibilities

Conduct comprehensive evaluations of patients' physical conditions.
Develop and implement individualized care plans to restore function and improve mobility.
Provide therapeutic interventions, exercises, and education to patients and caregivers.
Collaborate with interdisciplinary healthcare professionals to ensure coordinated care delivery.
Maintain accurate, timely, and compliant documentation of patient progress and outcomes.
Required Qualifications

Active California Physical Therapist (PT) License
Graduate of an accredited Physical Therapy program (Masters or Doctorate)
CPR (BLS) Certification
Valid drivers license and proof of auto insurance
At least 1 year of recent acute care or home health experience
Proficiency in physical therapy best practices and patient education
Excellent observation, documentation, and communication skills
Preferred Qualifications

Doctorate in Physical Therapy (DPT)
Previous home health experience in a community-based setting
Physical Demands

Frequent standing, walking, and patient handling required
Ability to lift/carry up to 50 lbs frequently and 100 lbs occasionally
Capability to push/pull up to 100+ lbs occasionally
Exposure to varied home environments, temperature changes, and equipment
