Location: Murrieta, CA
Work Setting: Hybrid (Field-based with administrative flexibility)
Schedule: MondayFriday, 8:00 AM 5:00 PM
Some Saturday availability required (810 times/year)
Compensation: $55$60 per hour
Travel Required: Yes (valid drivers license, insurance, and reliable vehicle required)

Position Summary

As a Physical Therapist (PT) in the Home Health team, you will plan, coordinate, and deliver rehabilitative services to patients in their homes. Your work will emphasize community-based care, education, and personalized treatment strategies that support patient independence, mobility, and quality of life.

Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions.

• Develop and implement individualized care plans to restore function and improve mobility.

• Provide therapeutic interventions, exercises, and education to patients and caregivers.

• Collaborate with interdisciplinary healthcare professionals to ensure coordinated care delivery.

• Maintain accurate, timely, and compliant documentation of patient progress and outcomes.

Required Qualifications

• Active California Physical Therapist (PT) License

• Graduate of an accredited Physical Therapy program (Masters or Doctorate)

• CPR (BLS) Certification

• Valid drivers license and proof of auto insurance

• At least 1 year of recent acute care or home health experience

• Proficiency in physical therapy best practices and patient education

• Excellent observation, documentation, and communication skills

Preferred Qualifications

• Doctorate in Physical Therapy (DPT)

• Previous home health experience in a community-based setting

Physical Demands

• Frequent standing, walking, and patient handling required

• Ability to lift/carry up to 50 lbs frequently and 100 lbs occasionally

• Capability to push/pull up to 100+ lbs occasionally

• Exposure to varied home environments, temperature changes, and equipment
