Purpose of Position:
Assess the needs of referred patients who have limited abilities established; develop and/or maintain the ability to complete daily functions and tasks concentrating on physical, mental, social and emotional sources of disabilities to improve quality of life. Will be responsible for occupational therapy services for Acute Care, long term care and Primary Care Clinic (outpatient) referrals. Will also be responsible for the direction, development, implementation, supervision, and ongoing evaluation of the activities program in our long term care facility, in our Acute Care swing bed program.

Essential Functions:
Provide therapy services defined in treatment plans to maintain daily living and work skills
Provide direction, development, implementation, supervision, and ongoing evaluation of the activities program in the long term care facility and swing bed program
Direct the contribution to the comprehensive care plan goals and approaches that are individualized to match the skills, abilities and interests/preferences of each resident/patient including scheduling of activities, both individual and groups, implementing and/or delegating the implementation of the programs, monitoring the response and/or reviewing the response to the program of the resident/patient and making revisions as necessary
Assist patients to improve decision-making, perceptual skills, sequencing, coordination, etc.
Recommend restorative or safety adaptations, provide patient education and guidance to family members
Cooperate with physicians, case managers, and other professionals in planning and conducting therapy leading the health restoration of the patient
Evaluate patient progress, report and document
Perform other duties as assigned
