What Youll Do

• Conduct in-home evaluations to assess patients physical health, strength, and mobility

• Develop and deliver customized care plans that support recovery and long-term wellness

• Educate patients and families on treatment goals, exercises, and safety practices

• Document progress and communicate with an interdisciplinary care team

• Help patients regain confidence, independence, and mobility in their daily lives

What Youll Need

• Active Physical Therapist license in Oregon (or eligibility prior to start date)

• Doctorate or Masters degree in Physical Therapy

• CPR certification

• Reliable transportation, valid drivers license, and auto insurance

• Prior PT experience is a plusbut not required. New graduates are encouraged to apply.
