Meet the resident's goals and needs and provides quality care by assessing and interpreting evaluations and test results; determining speech therapy treatment plans in consultation with physicians or by prescription Administer speech therapy treatments Complete discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to resident care conferences Assure continuation of therapeutic plan following discharge by instructing residents, families, and caregivers in home care programs; recommending and/or providing assistive equipment; recommending home health or follow-up programs What We're Looking For: You will be able to demonstrate composure, customer focus, patience, and the ability to gain trust and establish rapport with residents and your team. A strong desire and passion to work in an environment with people that are unwell, disabled, or living with dementia.
