Position: Full-Time Physical Therapist

Job Summary: As a Physical Therapist, you will play a crucial role in assessing, planning, and implementing individualized rehabilitation programs for our long-term care residents. Your expertise will help improve mobility, strength, and overall function, ensuring our residents achieve the highest possible quality of life.
Key Responsibilities:
- Conduct thorough assessments of patients’ physical conditions and needs.
- Develop and implement personalized treatment plans to address patients’ goals and needs.
- Provide direct therapeutic interventions, including manual therapy, exercise therapy, and modalities.
- Monitor and evaluate patient progress, adjusting treatment plans as necessary.
- Collaborate with a multidisciplinary team to provide holistic care.
- Educate patients and their families on treatment plans, exercises, and home care techniques.
- Document patient progress, treatment plans, and outcomes accurately and timely.
- Participate in care planning meetings and contribute to the development of care strategies.
- Ensure compliance with facility policies, procedures, and regulatory requirements.
Qualifications:
- Degree in Physical Therapy from an accredited institution.
- Current and valid Physical Therapy license in Minnesota.
- Previous experience in long-term care or a similar setting preferred.
- Strong knowledge of physical therapy techniques, modalities, and interventions.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team.
- Compassionate and patient-centered approach to care.
- Strong organizational and time-management skills.
