Job Summary: As a Physical Therapist, you will play a crucial role in assessing, planning, and implementing individualized rehabilitation programs for our long-term care residents. Your expertise will help improve mobility, strength, and overall function, ensuring our residents achieve the highest possible quality of life.

Key Responsibilities:

  • Conduct thorough assessments of patients’ physical conditions and needs.
  • Develop and implement personalized treatment plans to address patients’ goals and needs.
  • Provide direct therapeutic interventions, including manual therapy, exercise therapy, and modalities.
  • Monitor and evaluate patient progress, adjusting treatment plans as necessary.
  • Collaborate with a multidisciplinary team to provide holistic care.
  • Educate patients and their families on treatment plans, exercises, and home care techniques.
  • Document patient progress, treatment plans, and outcomes accurately and timely.
  • Participate in care planning meetings and contribute to the development of care strategies.
  • Ensure compliance with facility policies, procedures, and regulatory requirements.

Qualifications:

  • Degree in Physical Therapy from an accredited institution.
  • Current and valid Physical Therapy license in Minnesota.
  • Previous experience in long-term care or a similar setting preferred.
  • Strong knowledge of physical therapy techniques, modalities, and interventions.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Compassionate and patient-centered approach to care.
  • Strong organizational and time-management skills.
