Responsibilities:

Responsible for evaluating the patient's level of physical functioning and to provide training and/or assistive devices to eliminate or reduce areas of deficit. The therapist works with other professional staff to establish rehabilitation and maintenance goals and activities in compliance with applicable laws, regulations and Agency policies.

  • Develops a plan of care with patient and caregiver involvement and in collaboration with the nurse, identifying short and long term goals.
  • Evaluates, instructs and monitors specific functional exercise programs to increase strength, range of motion and coordination of upper extremities.
  • Teaches use of adaptive equipment to patient, caregiver and other appropriate health team personnel to assist the patient to achieve maximum independence.
  • Maintains communication with physician and other personnel involved in the patients care, through written reports, attendance at meetings and confirmed telephone calls.
  • Documents general information, provisional diagnosis, special precautions, description of treatment rendered on the progress notes for each visit and in the discharge summary.

Qualifications:

  • Current NY State Occupational Therapist License
  • Current Driver's License, required insurance and car available for work as required

Education:

  • Graduate of occupational therapy program approved by Council on Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association (AOTA) or graduate of an occupational therapy curriculum recognized by the World Federation of Occupational Therapists and is eligible for registration with the AOTA
  • 2 years of experience in general occupational therapy with current home health, medical, surgical or critical care
