**Job Title: Home Health Occupational Therapist Days**

**Location:** San Jose, CA

**Job Description:**

We are seeking a dedicated Home Health Occupational Therapist to join our team, delivering vital care to patients in their homes.

**Key Responsibilities:**

Provide occupational therapy in a home health setting.

Use Home Care Home Base for documentation.

Conduct start of care visits and manage caseloads.

Collaborate on individualized care plans.

Maintain accurate patient records.

**Qualifications:**

Valid OTR/L license.

12 years of home health experience.

Proficient in Home Care Home Base.

BLS certification.

**Contract Details:**

8week contract, 40 hours/week, day shifts.

Potential for extension and overtime.

RTO accepted.
