Our Opportunity

We are seeking a Occupational Therapist[PRN] whose primary responsibility is to provide occupational therapy services in accordance with the plan of care. Provide initial and on-going assessment and evaluation of patient response to therapy. Accurately document evaluation and treatments provided.

Key Responsibilities:

Provide services in accordance with accepted standards of practice for occupational therapy

Document all therapy interventions

Provide initial and ongoing assessment and evaluation of patient/family response to therapy

Responsible for attending all required facility meetings and in-services, including Medicare meetings, family meetings, care plans.

Develop and revise the plan of care in consultation with the physician and other care-team members

Complete documentation timely for all assessment and evaluation data, treatments, and patient's response to therapy intervention

Evaluate outcomes of care and complete the OASIS assessment accurately

Qualifications:

Minimum Education Required:

Bachelor of Science Degree in Occupational Therapy. State Licensure.

Current CPR Certification

Experience:

As applicable to assignment.
