Title : Patient Care Coordinator

Reports to: Nursing Director/Area Director of Infusion

Overview: As a Patient Care Coordinator, you will play a crucial role in ensuring seamless healthcare services for our patients. You will serve as the primary point of contact between patients, healthcare providers, and administrative staff, ensuring effective communication and coordination of care. Your dedication to patient welfare and your organizational skills will contribute significantly to our commitment to providing high-quality healthcare.

Responsibilities:

  1. Communication: Maintain regular communication with RN’s and patients to schedule appointments, provide information about medical procedures, and address any inquiries or concerns.
  2. Coordination of Care: Facilitate and coordinate patient care activities, including scheduling, medical records, and data entry to ensure timely and efficient healthcare delivery.
  3. Documentation and Record Keeping: Maintain accurate and updated patient records, including medical history, treatment plans, and insurance information, ensuring compliance with confidentiality regulations.
  4. Collaboration: Collaborate with healthcare providers, nurses, and administrative staff to ensure cohesive patient care delivery and resolve any issues or discrepancies promptly.
  5. Patient Advocacy: Advocate for patients' needs and rights, ensuring they receive appropriate care and support throughout their healthcare journey.

Requirements:

  • Proven experience in healthcare coordination or administrative roles within a clinical setting.
  • Excellent communication and interpersonal skills, with the ability to empathize and communicate effectively with patients and healthcare professionals.
  • Strong organizational skills and attention to detail to manage multiple tasks and prioritize effectively.
  • Proficiency in medical terminology and knowledge of healthcare regulations and compliance standards.
  • Ability to work independently as well as part of a collaborative team environment.
  • Familiarity with electronic health records (EHR) systems and patient management software (preferred).
