Utilizes a comprehensive evaluation based on accepted professional standards to assess patient needs and functional status. Devise and implement appropriate treatment plans to promote the highest level of patient self-sufficiency. Regularly re-assess plan efficacy, provide appropriate education to patients and families and progress patients toward goals and outcomes that reflect improved independence.

QUALIFICATIONS:

  • Prefer one year’s clinical experience but candidacy considered for any licensed clinician with abilities suited for Gaylord’s patient needs.
  • Required licensure and certification through the State of Connecticut for all disciplines. Exception is for Clinical Fellowship Year (CFY) in speech language pathology and audiology until certification is granted through ASHA. Therapeutic Recreation Staff must be certified or eligible for certification prior to employment and is required to maintain certification through NCTRC. Certification eligible staff must obtain CTRS certification within one year of hire. TR certification must be re-applied for every five years. For Physical and Occupational Therapy a current CT license or license eligibility required.

  • Must have required degree in specific profession from an accredited college or university recognized by the discipline’s professional organization. The minimal requirements for occupational, physical, recreation therapy are baccalaureate degrees in specific discipline. For audiology and speech language pathology, the minimum requirement is a Master’s degree in their respective discipline.

ESSENTIAL DUTIES & RESPONSIBILITIIES:

1. Provides direct patient care (evaluation and treatment).

CRITERIA:

  • Selects appropriate testing procedures and performs them in a competent and timely manner
  • Takes cultural diversity and population specific issues into consideration and utilizes this knowledge in the delivery of patient care
  • Involves patient and significant others in the development of appropriate long term goals and short term goals and provides an accurate estimation of time needed to reach those projected goals
  • Prioritizes, modifies and progresses treatment, goals and time needed appropriately in response to patient needs and response to care
  • Anticipates needs of each patient’s discharge including equipment, outpatient and clinic referrals, and patient/significant other training and education
  • Appropriately schedules patients based on needs and level of service
  • Meets productivity standards as assigned
  • Utilizes proper safety techniques and observations regarding self, each patient, their significant others and the environment
  • Exemplifies patient centered care as a priority (i.e. timeliness to each session, accommodation of patient personal preference)

2. Performs appropriate written communication.

CRITERIA:

  • Completes written evaluations within specifications for timeliness. Information should be accurate, concise, legible and complete
  • Completes progress, interim, and daily documentation in accordance with department and hospital standards. Updates progression towards goal attainment and identifies variances to goal attainment which includes a plan for treatment
  • Provides up to date documentation of therapy goals, treatment tasks/regimens to support staff and to staff providing sick or PTO coverage. (i.e. treatment cards and goal sheets)
  • Provides prescriptions for recommend equipment and referrals in a timely manner
  • Completes billing procedures accurately and within the allotted time frames

3. Interacts appropriately through clinical communication

CRITERIA:

  • Interacts with other therapists, support staff, medical personnel, nursing, care management, etc., to ensure that adequate feedback is provided
  • Practices effective 2 way communication with patients, hospital staff, listens openly, shares information, and asks questions as appropriate
  • Reports appropriately at patient focused meetings including team, TTC’s, and family meetings
  • Keeps supervisor informed of clinical issues, problems or concerns related to job role
  • Resolves conflicts appropriately and seeks help from supervisor, as appropriate
  • Proactive communication for therapists and assistants in communicating goal revision, treatment planning, LOS, room changes, need for discharge evaluation, etc.

4. Acts in a professional manner.

CRITERIA:

  • Complies with the Gaylord Hospital Code of Ethical Behavior and the Code of Ethics established by the discipline’s professional organization. Complies with state and federal law regarding practice of specific profession
  • Complies with all department and hospital policies and procedures
  • Participates in department performance improvement initiatives, goals, and risk management projects
  • Presents self appropriately in relation to position and job function
  • Assists with departmental coverage needs, as appropriate

5. Maintains state of the art training and education for job role.

CRITERIA:

  • Is knowledgeable about new techniques and trends in profession
  • Provides, participates and attends in-services, rounds and continuing education pursuits
  • Performs all other related duties, as assigned, requiring compatible skills and responsibilities

WORK SCHEDULE: Full Time, Monday - Friday, days, including occasional Saturdays
