Responsibilities Under the overall direction of the PM&R Manager, the Speech Therapist examines and evaluates patients and administers speech pathology interventions to patients with speech, voice, language, communication, cognitive, and swallowing impairments. The Speech Therapist works in collaboration with the referring provider. The Speech Therapist demonstrates competency in skills necessary to care for and manage primarily adult and geriatric age patients in acute care, outpatient, or acute inpatient rehabilitation setting. Essential Fuctions: Examines and evaluates all new pediatric, adolescent, adult and geriatric patients referred to Speech Pathology using data from the medical record and appropriate assessment techniques including pain assessment to identify the patient's needs or deficits for which Speech Pathology services are appropriate. Interprets the data and, using good judgment, establishes a comprehensive documented Plan of Care which includes mutually acceptable treatment goals with patient, family, or significant other. Re-evaluates the patient's treatment plan and modifies the plan/resets the goals when appropriate to reflect patient progress and rehabilitation potential. Coordinates the Plan of Care with PM&R Staff or inpatient rehabilitation unit staff and other appropriate members of the health care team in an interdisciplinary, collaborative manner to assure implementation and follow through of the treatment plan in a timely, effective, and organized manner. Administers Speech Pathology interventions competently, specific to the treatment setting according to recognized standards of appropriate care for the primarily adult and geriatric patients and observing all indications and contraindications of treatment. Demonstrates Speech Pathology competency by attaining desired treatment outcomes/goals and successful completion of the competency assessment process. Performs a learning assessment and identifies educational needs of the patient through appropriate assessment criteria and establishes/implements a teaching plan for the patient, family, and/or significant other. Patient education is interdisciplinary whenever appropriate. Addresses potential discharge needs and services of the patient early in the care planning process. Regarding inpatients and inpatient Rehab Unit patients, assists the patient, family, or significant other in preparation for discharge; communicates with Case Management about discharge plans, makes recommendations for continued Speech Pathology treatment via Inpatient Rehab., Home Care, or outpatient treatment, if appropriate, to further achieve rehabilitation goals, and provide continuity of care. Demonstrates the ability to work efficiently and provide quality care to a volume of an average of 8-10 patients/day according to patient diagnosis and treatment plan intensity. Schedule: Two Saturdays per month and weekday availability.
