About the job:
The Physical Therapist (PT) is responsible for evaluating and treating patients referred for physical therapy by a provider. In addition, the Physical Therapist communicates and collaborates with other caregivers to provide quality care and maintains accurate patient records in the EMR- TogetherCare powered by EPIC. The PT creates a caring and healing environment that keeps the patient and family at the center of care throughout their experience at Mount Carmel following the principles of our interdisciplinary care delivery system.
What you will do:
- Performs physical therapy evaluation, establishes and implements treatment plan for patients referred by a provider and assigned by the office staff or office coordinator. Documents accurately patient evaluation, treatment, progress, and discharge records. Maintains proper billing for the services provided.
- Delegates appropriate treatment procedures to the Physical Therapist Assistant, reviews, and co-signs their documentation.
- Communicates with appropriate health care providers on matters concerning patient treatment, continuity of care, and discharge planning.
- Maintains department readiness, i.e., supplies, linen, and equipment cleaning.
- Strive toward the achievement of advanced knowledge, skills and abilities for excellence within your scope of work.
What we are looking for:
- Minimum Education Requirement - Graduate of an accredited physical therapy program
- Licensure Requirement - Current license to practice physical therapy (PT) in the State of Ohio; Current BLS for Health Care certification.
- Effective communication skills.
- Physically able to perform strenuous activities, such as lifting and transferring patients
- Must hold a valid Ohio driver's license
