Physical Therapist, PRN

Position Summary : Under the vibrant leadership of our Center Manager, you'll provide essential physical therapy services to the wonderful participants of the Elderhaus , PACE Program. Your role will involve conducting thorough clinical evaluations and delivering personalized treatments across acute, restorative, and maintenance levels. Whether at our Day Center, in Nursing Homes, or in participants' homes, you'll make a real difference!

Essential Duties and Responsibilities:

  • Conducts Assessments : Performs initial and annual physical therapy assessments for all participants, coordinating with the Interdisciplinary Team to develop and update comprehensive Plans of Care.
  • Provides Treatment : Delivers physical therapy treatments, implements procedures to achieve goals, and grades activities. Confers with the Interdisciplinary Team on care issues.
  • Evaluates and Recommends : Assesses participants' needs, follows up during treatment, and recommends solutions. Evaluates the need for adaptive equipment and ensures proper fit.
  • Documentation and Reporting : Evaluates charts, documents progress, and maintains physical therapy notes according to charting requirements. Assists with data collection and prepares reports.
  • Supervision and Training : Directs and supervises Physical Therapy Assistants, performs competencies, and provides ongoing education and clinical supervision. Trains staff on body mechanics and equipment use.
  • Maintains Safety and Compliance : Ensures a safe working environment, follows policies and procedures, handles biohazard precautions, and participates in continuing education and quality improvement initiatives.

Minimum Education:

  • Physical Therapy License in North Carolina

Minimum Requirements:

  • CPR Certification ( Required )
  • 1 Year Working with Seniors (Preferred)

Other Requirements :

  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
