Hybrid Physical Therapist

Job Overview We are seeking a dedicated Physical Therapist to join our team. The ideal candidate will provide patient-centered care in various settings, Outpatient office setting and Short Term Care Facility Duties - Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons - Conduct assessments and develop individualized treatment plans for patients - Monitor patient progress and adjust treatment plans as needed - Educate patients and their families on at-home exercises and self-care techniques - Collaborate with other healthcare professionals to ensure comprehensive patient care Qualifications - Degree in Physical Therapy from an accredited program - Valid state licensure as a Physical Therapist - Experience in acute care, outpatient settings, or clinics preferred - Knowledge of medical terminology, orthopedics, physiology, and geriatrics - Strong interpersonal skills and the ability to work effectively in a team environment Join our dynamic team and make a difference in the lives of our patients through your expertise in physical therapy.
