Physical Therapist - Home Visits - Madison County

We offer a fantastic work/life balance, flexible schedules, family like company culture, advancement opportunities and more! *Role and Responsibilities* A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home following physician's orders. *Essential Functions* * Evaluate patient using diagnostic muscle, nerve, joint and functional ability tests and measures. * Prepare an assessment of the patient's condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment. * Recommends to physician, modification of plan of care as needed. * Documents clinical notes on the date of the visit into medical record and keeps all records and summaries up to date. * Treat patients to address impairments and functional limitations to reach maximum functional independence. * Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic and durable medical equipment (canes, walkers, wheelchairs, etc.) * Include and educate the caregivers and patient regarding treatment plan and goals. * Attend physical therapy staff meetings and in-service presentations. * Participate in case conferences. * Participate in the development and periodic revision of the physician's Plan of Treatment. * Assures appropriate safety practices and maintenance of equipment. * Other duties as assigned. *Qualifications and Requirements* * Graduate of an accredited physical therapy program; unencumbered PT license issued by the NYS Education Dept.. * One year of practical experience as a licensed physical therapist preferred Job Type: Full-time Pay: $80,000.00 - $90,000.00 per year Benefits: * 401(k) * 401(k) matching * Continuing education credits * Dental insurance * Employee assistance program * Family leave * Flexible schedule * Flexible spending account * Health insurance * Life insurance * Paid time off * Professional development assistance * Referral program * Retirement plan * Travel reimbursement * Vision insurance Medical Specialty: * Home Health Schedule: * 8 hour shift * Day shift * Monday to Friday Application Question(s): * This position requires ability to travel to see homebound patients in Madison County.
