UW Health is seeking a per diem/prn Physical Therapist for a per diem/prn job in MADISON, Wisconsin. Job Description & Requirements Specialty: Physical Therapist Discipline: Therapy Duration: Ongoing Employment Type: Per Diem Work Schedule: Per diem, day shift. Minimum requirement of one (1) weekend per month and one (1) holiday shift with the associated weekend per year. Hours may vary based on operational needs of department. We are seeking a Physical Therapist with acute care experience to: Provide consultative and rehabilitative treatment services to complex patients admitted to University Hospital in acute care settings. Work collaboratively with the interdisciplinary team to create comprehensive evidence-based treatment plans to maximize functional outcomes and discharge plans. Work within the context of patient- and family- centered care in a primary and specialty care academic medical center. Seek continual learning opportunities to advance one's own practice and educate others. Education: Minimum - Bachelor's degree in Physical Therapy is required. Work Experience: Preferred -Two (2) years prior relevant experience. Licenses and Certifications: Minimum - Licensure as a Physical Therapist in the State of Wisconsin is required. Basic Life Support (BLS) within three months of hire.
