Position Summary

The Bilingual Clinical Case Manager, under the supervision of the Director of Clinical Services and the Associate Director of Community Services, is responsible for delivering direct case management and therapeutic services in alignment with professional legal and ethical standards and the mission of the YMCA of San Francisco. This role requires the application of effective, solution-focused strategies and the ability to connect clients with appropriate resources. The Clinical Case Manager is expected to consistently attend the required meetings and training courses and to provide client services as outlined in the guidelines of the program.

Job Responsibilities

Direct Client Services

  • Provide clinical case management services to Mental Health Program clients, including those in:
    • Diversion Program of San Mateo
    • Juvenile Sexual Responsibility (JSR) Program of San Mateo
  • Delivering services such as:
    • Individual, group, and family counseling
    • Skills coaching
    • Community referrals and coordination of care
    • Crisis intervention and psychoeducation
    • Communication with service providers and juvenile justice systems
    • Client advocacy and parent education
  • Conduct services in client homes and community settings to increase accessibility and engagement.
  • Employ strength-based approaches in all interactions with clients and families.
  • Utilize JSR Curriculum and appropriate interventions to address problematic sexual behaviors in youth (JSR Program).

Collaboration & Teamwork

  • Actively participate as part of a Multi-Disciplinary Team (MDT), including BHRS, HSA, SMPD, JPD, L Program, and Community-Based Organizations, to coordinate services and treatment plans.
  • Collaborate with:
    • Police Officers (Diversion Program)
    • JSR Therapists, Probation Officers, and Social Workers (JSR Program).
  • Maintain effective communication with the clinical team and engage in individual and group clinical supervision per BBS requirements.

Documentation & Compliance

  • Maintain HIPAA-compliant clinical records using the electronic health record system (ECM).
  • Provide and document a minimum average of:
    • 12 direct service units/week for Diversion Program
    • 10 billable service units/week for JSR Program
  • Complete all service documentation within 24 working hours of service delivery.
  • Submit documentation into ECM database by end of day each Monday for the prior week.
  • Finalize reviewed documentation by end of day Monday to avoid revision duplication.
  • Track weekly service units via spreadsheet or planner.
  • Obtain client and guardian signatures by the second client contact.
  • Complete CANS assessments at case opening, case closing, and at 6- and 12-month intervals as applicable (Diversion Program).
  • Maintain CANS certification and ensure initial CANS assessments are entered in ECM within 714 days of initial meeting.
  • Ensure consent paperwork is renewed annually.

Crisis Management & Reporting

  • Immediately inform mental health leadership of client crises, potential reporting mandates, and complex clinical issues.
  • Submit all reports and letters to management for approval prior to delivery.
  • Check and respond to email within 24 hours.

Training & Professional Development

  • Maintain credential status as a Mental Health Rehabilitation Specialist (JSR Program).
  • Complete all required YMCA and Y-USA training, including:
    • ADP LMS safety training
    • Y-USA e-Learning (prior to first shift)
  • Attend the required staff meetings and professional development training courses.

Organizational Responsibilities

  • Uphold YMCA policies for safety, supervision, mandated reporting, and risk management.
  • Demonstrate the YMCAs Character Counts values of caring, respect, honesty, and responsibility.

  • Perform additional duties as assigned by the supervisor.

Work Environment and Physical Demands
This position operates in a dynamic community center environment that includes interaction with members of all ages, staff, and volunteers. The role requires the ability to remain on your feet for extended periods, navigate throughout the facility, and occasionally lift or move up to 25 pounds. The ability to respond quickly to emergencies, including administering basic first aid or assisting with facility evacuations, is essential. Noise levels can vary, and the environment may be fast paced during peak hours or special events.

Salary Description

The Bilingual Diversion & JSR Clinical Case Manager position offers a Salary of $31.93/ Hour. Based on experience, qualifications, and alignment with salary benchmarks for similar roles. Compensation is in accordance with grant funding approval and program budget requirements.

Minimum Qualifications

  • Over 18 Years old
  • Experience working with diverse youth and families in clinical and school settings.
  • Possess a clinical education background (MA/MS/MSW in counseling, social work,orpsychology) and professional registration or license: MFTI, ACSW, MFT,LCSW,PsyD, PhD.
  • Knows and implements the legal and ethical standards of practice, including child abuse reporting laws.
  • Possess sufficient clinical skills and insight necessary for affecting positive work with clients and their families.
  • Valid CA Driver s license and working car
  • Bilingual in Spanish.

Preferred Qualifications

  • MA or BA in social work, sociology, counseling, or psychology) AND four years of experience in a mental health setting.
