Pure Home Health Care is looking to bring Occupational Therapists home* Do you have a passion for helping patients develop, recover, improve, and maintain the skills needed for daily living. Pure Home Health Care (PHHC) is seeking to hire an Occupational Therapist to deliver care to patients in their place of residence. You'll help people fully engage in their lives by providing exercises, strategies, adaptations, adjustments, and tools that help them function more independently. *No two days are the same.* Every patient has different needs and challenges, and every residence provides a change of scenery. You'll be following the plan of care, and providing services that help reach goals set forth by their physician. *Duties:* Provide a variety of occupational therapy services to clients in their residence in accordance with physician's authorized plan of care Determine each patient's functional status and functional limitations in regards to activities of daily living, feeding, dressing, grooming, hygiene, communication, and homemaking skills When an occupational therapy assistant or certified home health aide is assigned, prepare written exercise program; supervises, instructs and directs in accordance with regulatory guidelines, Agency and accreditation standards Determine patient's equipment needs Evaluate and regularly re-evaluate the occupational therapy needs of the patient, and assess their continual care needs Initiate diagnostic, preventive and rehabilitative procedures as appropriate, and make referrals to other disciplines as indicated by the needs of the patient (or document rationale for not doing so) Observe signs and symptoms and report changes in the patient's condition to the appropriate health professionals as needed Develop, prepare and maintain individualized patient care progress records and submit documentation within 48 hours of visit Keep current in occupational therapy trends and knowledge and participate in in-service programs *Qualifications:* License to practice as an Occupational Therapist in the State of Michigan A degree in an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and the American Occupational Therapy Association, or eligibility for the National Registration Examination of the American Occupational Therapy Association At least one year of experience in a health care or rehabilitative facility with demonstrated competency in the practice of occupational therapy Current Michigan Operator's license, with reliable transportation and willingness to drive in inclement weather Willingness to maintain a flexible schedule and work the required hours, including evening and weekend hours as agreed upon Job Type: Part-time Pay: $45.00 - $55.00 per hour Benefits: * 401(k) * Professional development assistance Schedule: * Monday to Friday License/Certification: * Occupational Therapy License (Required) Work Location: In person
