What is Required? * Current PT license as required by state * CPR certification with American Heart Association or America Red Cross * 1 year of experience in a health care facility * Ability to meet physical demands of the job * Positive attitude * Dedication to quality patient care * Strong communication skills * Reliable transportation to perform job duties Responsibilities: * Provide physical therapy services to patients in a home health setting. * Utilize knowledge of physiology and anatomy to develop individualized treatment plans. * Conduct assessments and evaluations to determine patient needs and progress. * Collaborate with healthcare team members to ensure comprehensive care. * Document patient progress and maintain accurate records. * Demonstrate excellent communication skills with patients and their families. * Stay current with best practices and advancements in physical therapy. * Adhere to ethical and professional standards in all interactions.
