Quick Facts:

• Monday Friday schedule, 8:30 am 5 pm
• 6 10 patients per day at primary location
• 8 12 patients per day at secondary location one day per week
• SNF and acute swing bed coverage with occasional outpatient care
• One-on-one patient care in all settings
• Outpatient and acute split care includes orthopedic and neuro cases
• Aquatic therapy services provided
• Split coverage between two facilities
