Occupational Therapist

Welcome Homecare is a 5 star agency that has been serving the community of the greater Jacksonville area for more than 25 years. WHC is seeking an Occupational Therapist for our team in Jacksonville. We are interviewing for a Full-time position. Our OTs work under the order of the physician to provide skilled therapy and health teaching to patients and their caregivers in the home setting, as a team in conjunction with OTAs. Responsibilities: * Establishes therapy treatment plans to the patients as defined by the plan of care as prescribed by the physician. * Supervises and works in conjunction with Occupational Therapy Assistants. * Observing, recording activities and findings in the clinical record and reporting to the case manager and/or physician the patient's reaction to treatment and any changes in the patient's condition. * Collaborate appropriately with patients, caregivers, physicians, other therapists, nurses, aides, and other company staff on a regular basis based on the needs of the patient. * Attends case conferences on a weekly basis. * Performs discharge planning in conjunction with OTAs. * Accurately and thoroughly document patient care observations, interventions and evaluation on the day services are rendered. * Maintain and submit documents as required by the Company and/or facility. * Rotates On-Call weekends as census and need indicates Qualifications: * Must hold valid, clear, current licensure as a Registered Occupational Therapist in the State of Florida * Home Health experience highly desired * One year prior experience required. * Prior experience with electronic medical records preferred. * Current CPR certification is required. * Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
