Walker Healthforce is seeking a Home Health PT with 2+ years of experience to provide at-home, patient care for a healthcare client. This is a 13-week contract with potential for extension. 30 weekly visits within a 50-mile radius of client office.
ROLE DESCRIPTION:
Provide in-home PT to patients of all age groups including planning, implementing, and evaluating patient plans of care (POC) to restore and maintain wellbeing/quality of life. PT will design, organize, and execute treatment programs based on the medical referral and their evaluation in collaboration with an interdisciplinary team.
START DATE: ASAP
LOCATION: Forrest City, AR
COVERAGE: Stuttgart, Clarendon, Marvell, Holly Grove, Brinkley, Carlise, Devalls Bluff, West Memphis
SCHEDULE: 40/wk; 32 guaranteed hours
SHIFT EXPECTATIONS: No on-call or weekends
VISITS/WK: 30
RATE: $50/hr
BENEFITS: Mileage Reimbursement
CORE REQUIREMENTS:
1-2+ years of experience as a PT in an acute care or rehabilitation setting
1+ years of experience in home health settings
1+ years of experience with OASIS
REQUIRED CREDENTIALS:
Current state PT license
Proof of current/active CPR certification
Current, valid driver's license w/ insurance
REQUIRED EDUCATION:
BS, Physical Therapy from an American Physical Therapy Association approved program
PREFERRED EXPERIENCE:
1+ year of experience with Homecare Homebase (HCHB)
CORE RESPONSIBILITIES:
Perform clinical assessment to identify patient needs and appropriateness of patient
Document, prepare/review and complete a Plan of Care (POC), progress notes, phone orders and other clinical record documentation on an ongoing basis
Revise the POC based on evaluation, ongoing assessment data
Work in consultation with the physician and as part of an interdisciplinary team
Plan and provide appropriate treatment for neuromuscular, neurological and orthopedic disorders based on medical referral and therapy assessments
Assist/instruct patient/family in transfer techniques appropriate to patient abilities
Instruct on use of equipment such as walkers, wheelchair, canes, braces, prosthetic devices and appliances in a safe manner
Utilize appropriate therapy equipment specific to patient diagnosis and needs
Order appropriate rehabilitative equipment to meet patients' needs and adapt equipment, appliances to meet home limitations/requirements
Provide appropriate patient/family education regarding therapeutic procedures based on the patient's age, condition, culture and learning readiness
Perform and oversee other team members, when applicable, in the performance of patient care that is consistent with patient's needs and desires
Apply concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
Accept clinical assignments that are consistent with education and competence to care for patients
Perform other related duties as assigned or requested
